Out-of-network benefits:
Many insurance policies have "out of network benefits" that permit you to see a licensed healthcare provider that is not in network. I am happy to provide you with a superbill that you can submit to your insurance provider. It is recommended that you contact your insurance and inquire about out of network benefits.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
By law, health care providers must give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
Note: A Good Faith Estimate is intended for you to be aware of estimated fees. The estimate does NOT mean you need to make a commitment to the length or frequency of therapy sessions.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur.
If you are billed at least $400 more than the sum provided in your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.

I require 24 hour notice for cancellations and/or rescheduling. There is a $50 charge for the first missed appointment, or providing less than 24 hour notice. There is a $100 charge for each additional missed appointment or late cancellation. There is no charge for cancellations due to illness, family emergencies, inclement weather. Before your first appointment, I will invite you to my "Client Portal". There, you will have the ability to complete the forms needed prior to your first appointment. These may take about 30 minutes to complete. They are designed to inform you about my practice, to provide necessary legal information, and to inquire about what is bringing you in. Please complete these to your best ability and save them to the Client Portal.
